Downriver Health provides eligibility screening as a courtesy service to assist families in understanding whether they may qualify for the Michigan Medicaid Home Help Program. Completion of an eligibility screening with us does not guarantee approval for program benefits.

All final eligibility determinations, service authorizations, and approved care hours are made exclusively by the Michigan Department of Health and Human Services (MDHHS) based on their assessment criteria, applicable state law, and available program funding. We make no representation or warranty that any individual will be approved.

The Michigan Home Help Program is administered solely by MDHHS. Downriver Health is an independent home care agency that assists families with the enrollment process and employs approved caregivers as W-2 workers. We are not an agency of the State of Michigan and are not affiliated with or endorsed by MDHHS.

MDHHS retains sole authority over: (a) whether a care recipient qualifies for the program; (b) the number of authorized care hours per month; (c) which tasks are approved on the individual’s MSA-1904 care plan; and (d) any changes or terminations to program benefits.

You may use our website for lawful purposes only. You agree not to:

• Attempt to gain unauthorized access to any part of the site or our systems.
• Transmit malicious code, spam, or unsolicited communications.
• Reproduce, distribute, or create derivative works from site content without our written permission.
• Use the site in any manner that could impair its operation or harm other users.

To the fullest extent permitted by Michigan law, Downriver Health, its owners, employees, and agents shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising out of or related to your use of our website or services, including but not limited to:

• Denial or loss of program benefits by MDHHS.
• Delays in the enrollment or approval process.
• Changes to program rules, funding, or authorized hours.
• Reliance on information provided on this website that is subsequently changed.

Our total liability for any claim arising out of these Terms shall not exceed the amount you paid us for services in the three months preceding the claim, or $100, whichever is greater.

When you submit a form or otherwise consent to receive text messages from us, you authorize Downriver Health to send SMS messages to the phone number you provide. Message frequency varies. Message and data rates may apply. You may reply STOP to opt out at any time and HELP for assistance.
