By opting in to receive SMS messages from Downriver Health, you agree to these SMS Terms and Conditions. Please read them carefully before consenting to receive text messages.

1. Program Description

Downriver Health operates an SMS messaging program to communicate with clients, family members, and caregiver applicants. Messages sent through this program may include:

  • Appointment confirmations and reminders
  • Service schedule updates and changes
  • Caregiver assignment notifications
  • Medicaid Home Help program information and eligibility updates
  • Responses to inquiries you submit to us
  • Important service alerts and safety notices

This program is operated by Downriver Health, located at 100 W Big Beaver Rd, Ste 200, Troy, MI 48084.

2. How to Opt In

You may opt in to receive SMS messages from us through any of the following methods:

  • Web form — by providing your mobile phone number and checking the SMS opt-in checkbox on our contact, intake, or caregiver application form.
  • Verbal consent — by providing express verbal consent during a phone consultation and confirming your mobile number.
  • In-person — by signing a written consent form during an in-home assessment or office visit.

Consent to receive SMS messages is not a condition of receiving our services. You may receive services without consenting to SMS communications.

3. Message Frequency

Message frequency varies based on your service status and the nature of your inquiries. Typical message volume is approximately 2 to 15 messages per month. Active clients receiving regular services may receive more frequent messages around scheduling changes or care updates.

We will not send unsolicited promotional messages. Messages are limited to service-relevant communications.

4. Message and Data Rates

Message and data rates may apply. Standard SMS and data rates charged by your mobile carrier will apply to messages sent and received. Downriver Health does not charge a fee for SMS messages. Contact your mobile carrier for details about your plan's SMS and data charges.

5. How to Opt Out (STOP)

You may opt out of receiving SMS messages from us at any time:

  • Reply STOP to any SMS message you receive from us.
  • Contact us at (248) 688-0966 and request to be removed from SMS communications.
  • Email us at info@downriverhealth.com with your opt-out request and mobile number.

After opting out, you will receive one final confirmation message. No further SMS messages will be sent unless you opt back in. Opting out of SMS does not affect your services.

7. Messaging Platform

We use Grasshopper, a third-party communication platform, to send and receive text messages on behalf of Downriver Health. Use of Grasshopper does not change your rights or our obligations under these SMS Terms or our Privacy Policy. Messages you exchange with us through Grasshopper may be processed and stored by that provider subject to its own data practices.

8. Supported Carriers

Our SMS program supports all major U.S. mobile carriers, including but not limited to: AT&T, Verizon, T-Mobile, US Cellular, Cricket Wireless, Boost Mobile, Metro by T-Mobile, Consumer Cellular, and other regional and MVNO carriers.

Carrier support for SMS programs is not guaranteed. Carriers are not liable for delayed or undelivered messages.

9. Privacy

Mobile information and SMS opt-in consent will not be shared with third parties or affiliates for marketing or promotional purposes. Your mobile phone number and SMS consent are collected solely to facilitate communications described in this program. They will not be sold, transferred, or used for any purpose unrelated to your care or service relationship with Downriver Health.

10. Changes to These Terms

We may update these SMS Terms and Conditions from time to time. The current version will always be available at this URL. Continued use of our SMS service after changes take effect constitutes acceptance of the updated terms.
